Position Overview:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in Physical Medicine & Rehabilitation (PM&R) to join our growing team in Charlotte, NC. In this full-time role, you will provide high-quality, patient-centered care to geriatric and rehabilitation patients within a skilled nursing facility (SNF).

Key Responsibilities:
  •  Deliver comprehensive PM&R care to geriatric and post-acute patients 
  •  Collaborate with interdisciplinary teams including nursing, therapy, and facility administration 
  •  Develop and implement individualized treatment and preventative care plans 
  •  Build strong relationships with patients, families, and facility staff 
  •  Lead initiatives to reduce hospitalizations and improve functional outcomes 
  •  Ensure continuity of care and clinical excellence across the facility

Compensation & Benefits

  •  Active North Carolina Nurse Practitioner License 
  •  DEA License
