Kinetic Medical Group (KMG) is looking for a Full-Time Nurse Practitioner in Physical Medicine & Rehabilitation to join our growing team and serve patients in a skilled nursing and rehabilitation facility in Belmont, NC.

What You'll Do
As a Physical Medicine & Rehabilitation Nurse Practitioner, you will:
- Provide comprehensive medical care to geriatric and rehabilitation patients within the SNF setting
- Evaluate and manage patients' rehabilitation and physical medicine needs
- Develop and monitor individualized treatment plans
- Collaborate closely with nursing, physical therapy, occupational therapy, speech therapy, administration, and families
- Identify and address medical issues before they become emergencies
- Participate in preventative care and quality-improvement initiatives
- Help reduce avoidable hospitalizations and improve patient outcomes
- Communicate effectively with patients, families, and the interdisciplinary care team
- Serve as a clinical resource and trusted partner to the facility
- Practice with a high degree of autonomy while having support from the KMG team
