Position Overview:

We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in Physical Medicine & Rehabilitation (PM&R) to join our growing team in Charlotte, NC. In this full-time role, you will provide high-quality, patient-centered care to geriatric and rehabilitation patients within a skilled nursing facility (SNF).

Key Responsibilities:

Deliver comprehensive PM&R care to geriatric and post-acute patients
Collaborate with interdisciplinary teams including nursing, therapy, and facility administration

Lead initiatives to reduce hospitalizations and improve functional outcomes
